Coordinator Registration

To obtain a username and password to gain access to Physical Fitness data, District Coordinators must download and complete a Contact Survey. Fax your survey to (408) 776-7696.

Submission File Formats

To submit your data to EDS, data files must be properly formatted. Text files must follow the School Layout and Student Layout rules. EDS also provides Data Entry Templates in Excel and Access formats. Fitnessgram software users, please follow these instructions.

Data submission allows districts to securely submit their fitness assessment results via the Internet.

Districts may access Individual Student Reports (ISRs) and importable data files for past test results.
