Coordinator Registration

To obtain a username and password to gain access to Physical Fitness data, District Coordinators must download and complete a Contact Survey. Fax your survey to (408) 776-7696.

Submission File Formats

To submit your data to EDS, data files must be properly formatted. Text files must follow the School Layout and Student Layout rules. EDS also provides Data Entry Templates in Excel and Access formats. Fitnessgram software users, please use our FitnessGram Instruction Kit (ZIP - 30KB).

Data Submission

Data submission allows districts to securely submit their fitness assessment results via the Internet.

Templates for Excel and Access data uploads are avilable now, so that you may prepare your data for upload. Use the record layout files above to prepare ASCII Text uploads.

 

District Reports

Districts may access Individual Student Reports (ISRs) and importable data files for past test results.

Login Now

For security reasons, do not e-mail student data files.
