Download one of the three templates below based on your application and version preference.

To avoid having the document open inside your browser, you may need to save the file by right-clicking on it and selecting "Save Target As" or "Save Link to Disk".

Access 2000   |   Excel 97 / 2000

The data entry field lengths and parameters for populating these templates are specified in the School and Student layout files.
